1- Proof of identity with date of birth such as: Passport, local driving license OR national identification paper stamped by Canadian customs.
2- Form IMM 1017 (Medical report).
3- Asylum seeker document (Brown paper).
You get an appointment the same day of your call for a complete and precise medical examination by our licensed panel physicians.

Don’t fast, you can eat. We suggest drinking water for your urine sample.
